Terms
Terms Overview
BridgeCare Medicine provides private physician advisory, interpretation, preparation, and family decision support for non-emergency medical questions after serious illness.
Advisory Service Only
BridgeCare helps families understand available records, clarify medical language, and prepare better questions for the treating team already caring for the patient.
BridgeCare does not replace the treating physician, hospital team, primary care physician, facility clinician, or emergency services.
Not Emergency Care
Do not use BridgeCare for emergencies, urgent symptoms, rapid clinical changes, or time-sensitive medical decisions. For urgent concerns, call 911, contact the treating clinician, contact facility staff, or go to the emergency department.
No Relationship From Website Inquiry Alone
Submitting a website inquiry does not create a patient-physician relationship. A service relationship begins only after fit screening, scope confirmation, consent, payment, and acceptance by BridgeCare.
No Outcome Guarantees
BridgeCare does not guarantee a diagnosis change, treatment change, hospital outcome, discharge outcome, medication change, insurance coverage, or clinical outcome.
Prescribing
Unless explicitly stated in a signed service scope, BridgeCare does not provide prescriptions. The standard public service is designed around advisory interpretation and preparation.
Records And Accuracy
Clients are responsible for providing accurate and complete information through the approved secure workflow. BridgeCare's interpretation is limited by the information available.
Do not send protected health information, records, medication lists, or urgent symptom details by ordinary email.
SMS Terms
By opting into SMS from BridgeCare Medicine through a web form, phone call, professional referral, or other consent-based medium, you agree to receive non-emergency SMS messages from BridgeCare Medicine. These may include inquiry responses, callback coordination, scheduling-related updates, referral follow-up, and administrative messages.
Message frequency varies. Message and data rates may apply. Reply HELP for assistance or STOP to opt out at any time. For assistance, contact hello@bridgecaremedicine.com or visit bridgecaremedicine.com.
Do not send protected health information, medical records, medication lists, diagnoses, dates of birth, medical record numbers, urgent symptom updates, or emergency information by text message. For urgent concerns, call 911, contact the treating clinician, contact facility staff, or go to the emergency department.
See the BridgeCare Medicine privacy policy at https://bridgecaremedicine.com/privacy.html.
